New Patient Information
Welcome to our Practice!
Please ensure that you have the following before you call for your appointment. Our Doctor personally reviews each and every new patient’s records before we schedule you so that we can provide you with the most complete care.When you call you will need:
- The names and telephone numbers of the doctors who are treating you. You are going to need to call or fax them to authorize them to release your records to us. We need copies of your records faxed over ATN: NEW PATIENT BEFORE we can schedule your appointment. Our fax # is 941-923-2520
- Your primary care doctors information ( if you have one)
- Your insurance information
- What medications you are talking including the doses and how many you take of each one?
- Any recent MRI Cat Scan or X-ray reports or the name of the facility where you had the study done.
We will make every effort to get you scheduled and in to see the Doctor as soon as possible. We must have all the information above to begin the process, the more prepared we are, the better we can serve you.
Our office strives to provide our patients with the highest quality care. We allow enough time for you to meet with the Doctor for a complete evaluation. Therefore, we require a full 24 hours notice for any cancellations. If proper notice is not given , a $100.00 cancellation fee will be assessed to your account and will be payable by you within 15 days.
